Transvaginal Ultrasound

What Is a Transvaginal Ultrasound?
A transvaginal ultrasound (also known as endovaginal sonography) is a specialized imaging test that provides a detailed view of a woman’s uterus, ovaries, endometrium, and pelvic structures.
Unlike abdominal ultrasound, it uses a slender, high-frequency probe inserted gently into the vagina, allowing closer proximity to pelvic organs and therefore clearer, more accurate images.
At Saga Clinic in Tirana, Dr. Eser Agar performs transvaginal ultrasound examinations using advanced 3D and Doppler ultrasound systems for optimal diagnostic accuracy and patient comfort.
Why Is It Important?
Transvaginal ultrasound is a key diagnostic tool in modern gynecology, allowing early detection of numerous conditions that may not be visible during a standard pelvic exam.
It is used to evaluate:
- Abnormal uterine bleeding or menstrual irregularities
- Uterine fibroids, endometrial polyps, and adenomyosis
- Ovarian cysts or tumors
- Pelvic pain or suspected infection
- Early pregnancy and miscarriage evaluation
- Endometrial thickness in postmenopausal women
- Follicular tracking during fertility treatments
Because of its proximity to pelvic organs, transvaginal ultrasound provides high-resolution imaging with excellent diagnostic precision.
When Should It Be Performed?
Your doctor may recommend a transvaginal ultrasound in the following situations:
- Abnormal bleeding: To assess endometrial thickness or detect structural causes.
- Pelvic pain: To identify cysts, fibroids, or infection.
- Infertility assessment: To evaluate ovulation, endometrial development, and uterine structure.
- Postmenopausal bleeding: To rule out endometrial hyperplasia or malignancy.
- Routine annual check-up: As part of a comprehensive gynecological evaluation.
The exam can be done at any time, but the first 7–10 days of the menstrual cycle usually provide the best visualization.
How Is the Procedure Performed?
- The patient lies in a gynecological position.
- A covered, lubricated ultrasound probe is gently inserted a few centimeters into the vagina.
- Real-time images of the uterus, ovaries, and endometrium are displayed on the monitor.
- Doppler mode may be used to evaluate blood flow when necessary.
- The entire examination usually takes 5–10 minutes and is painless.
No anesthesia is required. Most women describe only mild pressure, similar to a pelvic examination.
What Can Be Detected?
A transvaginal ultrasound can help diagnose:
- Uterine fibroids (myomas) and their size, location, and number
- Endometrial polyps or thickening (hyperplasia)
- Ovarian cysts, endometriomas, or tumors
- Pelvic inflammatory disease (PID)
- Congenital uterine anomalies (septate or bicornuate uterus)
- Early intrauterine pregnancy or ectopic pregnancy
- Postoperative evaluation after gynecologic procedures
Preparation Before the Exam
- No fasting or special preparation is required.
- Empty your bladder before the examination for better image quality.
- Avoid vaginal creams or douching on the day of the test.
- The exam can be safely performed at any time except during heavy menstrual flow (unless clinically indicated).
After the Examination
The results are available immediately.
Dr. Eser Agar discusses the findings in detail, explaining what was observed and whether further tests or treatment are required.
A written or digital report can be provided for your medical records or for your referring physician.
