Pelvic Pain and Infections

Pelvic pain and gynecological infections are common concerns among women and can have a significant impact on reproductive health and fertility. Pelvic discomfort may stem from hormonal imbalance, structural abnormalities, inflammation, or chronic infection—each requiring accurate diagnosis and tailored treatment.
At Saga Clinic, we evaluate pelvic pain comprehensively using advanced imaging, laboratory testing, and ESHRE-aligned diagnostic protocols to determine the exact underlying cause.
What Is Pelvic Pain?
Pelvic pain refers to discomfort located in the lower abdomen and pelvis. It may be acute, lasting hours or days, or chronic, persisting for more than six months.
Pelvic pain can interfere with daily activities, sexual function, and fertility potential.
Common Causes of Pelvic Pain
1. Endometriosis
A leading cause of chronic pelvic pain and infertility, endometriosis occurs when endometrial-like tissue grows outside the uterus.
Symptoms may include:
- Painful menstruation
- Pain during intercourse
- Chronic pelvic pain
- Bowel or urinary discomfort
Endometriosis may affect ovarian reserve, tubal function, and implantation potential.

2. Pelvic Inflammatory Disease (PID)
PID is an infection of the uterus, fallopian tubes, or ovaries, often resulting from sexually transmitted infections or untreated bacterial infections.
If not treated promptly, PID can lead to:
- Tubal scarring
- Adhesions
- Chronic pain
- Increased risk of ectopic pregnancy
- Infertility
3. Ovarian Cysts
Functional or pathological cysts can cause unilateral pelvic pain, bloating, or discomfort.

4. Fibroids (Myomas)
Uterine fibroids—especially submucosal or large intramural types—may cause pelvic pressure, pain, and infertility.
→ Read More --> Uterine Fibroids
5. Adenomyosis
A condition where endometrial tissue grows into the uterine muscle, leading to heavy bleeding and painful menstruation.
6. Adhesions (Scar Tissue)
Pelvic adhesions may form after:
- PID
- Prior surgery
- Endometriosis
- Cesarean section
They can cause chronic pain and tubal obstruction.
7. Gastrointestinal or Urological Causes
Conditions such as IBS, urinary infections, kidney stones, or hernias may mimic gynecologic pelvic pain and require differential diagnosis.
Gynecological Infections and Fertility
Vaginitis and Cervicitis
Infections of the vagina or cervix may be caused by bacteria, yeast, or sexually transmitted pathogens.
Common symptoms include:
- Burning
- Discharge
- Odor
- Itching
- Pain during intercourse

Sexually Transmitted Infections (STIs)
Chlamydia and gonorrhea are particularly important because they can cause silent damage to the fallopian tubes, resulting in tubal factor infertility.
Chronic Endometritis
Persistent low-grade infection of the endometrium can affect:
- Implantation
- IVF success
- Pregnancy maintenance
Diagnosis may require biopsy, culture, or hysteroscopy.
