Female Fertility Evaluation

Female-Fertility-Evaluation

Understanding Female Fertility

Female fertility depends on a complex balance of hormones, healthy ovaries, open fallopian tubes, and a receptive uterus.
When any part of this system is disrupted, conception may become difficult.

At Saga Clinic Tirana, Dr. Eser Agar performs a complete, step-by-step evaluation to identify the exact cause and guide couples toward the most effective treatment.

Why a Female Fertility Evaluation Is Important

Many women do not experience any symptoms until they try to conceive. Early evaluation helps us:

  • Detect hormonal disorders
  • Assess ovarian reserve
  • Identify ovulation problems
  • Evaluate the uterus and fallopian tubes
  • Plan treatment before fertility declines further

Early diagnosis is key to choosing the safest and most successful approach.

When Should a Woman Seek Fertility Evaluation?

You should be evaluated if:

  • You are under 35 and have been trying to conceive for 1 year
  • You are over 35 and have been trying for 6 months
  • You have irregular periods
  • You have a history of polycystic ovaries (PCOS)
  • You have known fibroids, cysts, or endometriosis
  • You experience severe menstrual pain
  • You had previous pelvic infection or surgery

Components of Female Fertility Evaluation at Saga Clinic

1. Detailed Medical & Reproductive History

We take time to understand:

  • Menstrual cycle patterns
  • Pregnancy history
  • Past surgeries or infections
  • Symptoms such as pain or abnormal bleeding
  • Lifestyle factors and stressors

This forms the foundation of accurate diagnosis.

2. Hormonal Testing

Hormones control ovulation, egg development, and menstrual regulation.
We analyze:

  • FSH, LH
  • AMH (Anti-Müllerian Hormone) – ovarian reserve
  • Estradiol (E2)
  • Progesterone
  • Prolactin
  • TSH and thyroid panel
  • Androgens (if PCOS is suspected)

These results show whether the ovaries are functioning normally.

3. Ovarian Reserve Assessment

Ovarian reserve reflects the number and quality of remaining eggs.

Methods include:

  • AMH level
  • Antral Follicle Count (AFC) on transvaginal ultrasound
  • Day-3 FSH/E2 levels

This helps determine the most suitable treatment and whether time is a critical factor.

4. Ultrasound Examination

A high-resolution transvaginal ultrasound is performed to assess:

  • Ovaries (follicles, cysts, PCOS structure)
  • Uterus (shape, thickness, fibroids, adenomyosis)
  • Endometrial lining
  • Any anomalies affecting implantation

Ultrasound is one of the most important tools in fertility assessment.

5. Ovulation Evaluation

We confirm whether ovulation is happening regularly by:

  • Mid-luteal progesterone level
  • Follicular tracking via ultrasound
  • Cycle pattern interpretation

Irregular or absent ovulation is a common and treatable cause of infertility.

6. Fallopian Tube Assessment

Healthy, open tubes are essential for natural conception.

We evaluate tube patency using:

  • Hysterosalpingography (HSG)
  • HyCoSy (ultrasound contrast)
  • Laparoscopy (if needed)

Tubal blockage or damage significantly affects fertility and guides treatment choices.

7. Uterine Cavity Assessment

The uterine environment must support embryo implantation.

We may perform:

  • Saline infusion sonography (SIS)
  • Diagnostic hysteroscopy
  • Evaluation for polyps, fibroids, adhesions, or congenital anomalies

These issues can often be corrected with minimally invasive procedures.

8. Additional Tests (If Needed)

  • Genetic testing
  • Blood clotting panels
  • Autoimmune markers
  • Endometrial biopsy (for inflammation or receptivity problems)

Used selectively depending on the clinical picture.

Treatment Planning

After all evaluations are completed, Dr. Eser Agar creates a personalized plan which may include:

  • Cycle regulation
  • Ovulation induction
  • Treatment of hormonal disorders
  • Surgical correction of structural issues
  • IUI or IVF if indicated

The goal is always the least invasive and most effective approach first.
